Safeguarding Concern Form

If you have a safeguarding concern, please contact a member of the Regional Safeguarding Team , and complete a concern form so we have a full record of your concerns. Anyone can fill out this form. Don't worry if you don't have all of the information – just fill in what you do know.

Event risk assessments

Risk assessment should be carried out for church activities and notification of events with church groups off the premises must be given to the church safeguarding officer PRIOR to the agreement for any event or off site activity.

If the activity is unusual or considered to be high risk the church safeguarding officer will contact the circuit safeguarding officer in order that it can be ratified or any queries raised. (See attached documents for an event risk assessment pro form and an example.
